One day you are crushing your workouts and sleeping like a baby; the next, you are waking up drenched in sweat, staring at the ceiling, and wondering where your motivation went.


If you are in your late 30s, 40s, or early 50s, this isn’t a personal failing. It is perimenopause—and as a trainer and nutrition advisor, I want you to know that these symptoms are not "all in your head." They are in your biology. In her game-changing book Next Level, exercise physiologist Dr. Stacy Sims pulls back the curtain on what is actually happening. It’s not just “hormones” behind your hot flashes, brain fog, and sudden mood shifts—it is how those hormones talk to your brain.


The Estrogen-Serotonin Connection


We often think of estrogen simply as a reproductive hormone, but it is actually a master regulator of your central nervous system. Estrogen acts like a helper for your brain's feel-good chemicals—specifically serotonin (which regulates mood, sleep, and body temperature) and dopamine (your motivation modulator).


During perimenopause, estrogen doesn’t just smoothly glide downhill; it goes on a wild rollercoaster.


When estrogen spikes and plunges, your serotonin levels drop right along with it. Without that stabilising serotonin, your brain loses its natural "calm." This sudden chemical dip is a physiological trigger for the anxiety, irritability, and unprovoked mood swings that many women suddenly experience.


The Science Behind Your Symptoms


Dr. Sims explains the mechanics behind three of the most common complaints I hear from clients:

  • Hot Flashes & Night Sweats: Your hypothalamus is your brain’s thermostat. It relies on steady estrogen and serotonin to know what temperature is "normal." When these chemicals drop, your thermostat’s comfort zone shrinks. A tiny, normal temperature change suddenly triggers a panic response: your brain thinks you are overheating, dilating your blood vessels and activating sweat glands to cool you down.

  • The "Invisible Threat" (Cortisol): Estrogen normally keeps your stress hormone, cortisol, in check. As estrogen declines, cortisol goes wild. This leaves you in a constant, low-grade "fight-or-flight" state—feeling wired but tired, as if you are running from an invisible threat.

  • Brain Fog & Joint Aches: Estrogen has a natural pain-relieving effect and supports cognitive processing. When it drops, your pain tolerance decreases (making workouts feel harder and joints achier), and memory retrieval slows down.


How We Pivot


As an active woman, your instinct might be to work harder—to eat less, do more cardio, and push through the fatigue. Please don't.


When your body is already under hormonal stress, extreme dieting or fasted cardio only spikes your cortisol higher, worsening your symptoms and stalling your progress.

Instead, we have to change the stimulus. In my coaching, we focus on Dr. Sims's core principles: lifting heavy weights to stimulate muscle growth, utilising short, sharp interval training, and timing your protein intake to support your brain and muscles.


For clients new to fitness, we don't start with heavy barbells or high-intensity intervals right away. Pushing a stressed, perimenopausal body too hard, too fast only spikes cortisol and worsens fatigue. Instead, we build a resilient foundation first using targeted mobility and strength-initiation protocols.


We start by unlocking tight hips, thoracic spine (upper back), and shoulders through daily active mobility sequences. This improves joint range of motion and ensures your body can move safely. Alongside mobility, we introduce bodyweight stability and body weight resistance exercises—such as glute bridges, planks, and squats with a light dumbbell. This phase coaxes your nervous system into adapting, wakes up sleeping muscles, and establishes proper movement patterns. By introducing consistency and mastering fundamentals, we prep your joints and build the baseline strength required to safely transition to heavy lifting and HIIT without injury.


You do not have to just "endure" this transition. By understanding the science, we can train smarter, feed our brains what they need, and keep you performing at your absolute best.
